Business Developer

Your Business Developer is a person who monitors the proper execution of the contract with your company. In any matters related to the cooperation with Medicover within the scope of medical services for your employee, please contact your Business Developer. Any letters, inquiries and comments should also be addressed to your Business Developer.

The clients from Warsaw should call 22 592 70 70.  In any other place in Poland, please contact the regional Sales Department or send an e-mail at opiekunklienta@medicover.pl (one e-mail address for all the regions).

Changes in the list of entitled/insured persons

The annex to the Medical Care Contract /Group Insurance Contract is a list of your employees who will be covered by the contract. The list should be updated each time when there are any changes made.
Please remember to inform Medicover about:

  • Engaging a new employee who will be covered by the Medicover healthcare plan
  • Adding to the Medicover healthcare plan an employee who has not been included in it so far
  • Changing a type of the Medicover card for specific employees
  • Intending to end the membership of certain employees in Medicover

  • The company-specific E-Member application
    The Electronic System of Corporate Customer Service (E-Member) is intended for the registration of members in Medicover through the special website. You are also provided access to online reports. Learn more>>

    • List of entitled persons (.xls 198 KB)
      The form is intended for the registration of the persons entitled to use medical services provided by Medicover Sp. z o.o. Please complete it carefully and then submit it to the Sales Department or the Corporate Customer Service Department.
    • List of insured persons (.xls 186 KB)
      The form is intended for the registration of persons entered for the insurance in the Insurer’s system. Please complete it carefully and then submit it to the Sales Department or the Corporate Customer Service Department

Medicover, as a healthcare facility, is obliged to collect and store the data concerning patients provided for in the law (according to the Ordinance of the Minister of Health of 10 August 2001 on the types of medical documentation in healthcare facilities, Journal of Laws No. 88, item 966, clause 6, point 1). Therefore, please send us the complete and accurate data concerning your employees.
