Patient in the Care Centre – what is this programme about?

We would like to provide you with the access to your Primary Care Doctor always when you need this, and always in the most convenient way.
As part of the Patient in the Care Centre Programme, your Primary Care Doctor closely cooperates with other doctors from the healthcare team and with a nurse — the Healthcare Coordinator who is available on the phone. Your Healthcare Coordinator will arrange an appointment for you, or take care of those cases where the doctor's intervention is not indispensable.

Continuous medical care provided by your Primary Care Doctor and your nurse who know you well and are aware of your health problems ensures safe and effective treatment. This also provides you with individual solutions and all support in keeping good health.
Please choose the Primary Care Doctor you trust (internist, paediatrician, or family practitioner), contact him or her with your health problems, and ask your Healthcare Coordinator for advice and other healthcare services.

Please note: should you need medical help, please call your Healthcare Coordinator. Your Healthcare Coordinator ensures that the help you need reaches you on time. Please contact your Healthcare Coordinator at the Customer Care Centre: 500 900 500.
